We are excited to announce our Street Outreach Specialist opening with our Street Outreach and Engagement Program in Newark, NJ! The Street Outreach Specialist is responsible for engaging individuals experiencing homelessness, building trusting relationships, and connecting participants to housing, behavioral health care, medical services, and other essential community resources. Using a trauma-informed, person-centered approach, this role helps improve the health, stability, and well-being of some of Newark’s most vulnerable residents.
Duties and Responsibilities of a Street Outreach Specialist:
- Conduct behavioral health assessments using evidence-based assessment tools to determine appropriate behavioral health interventions, levels of care, and referrals.
- Conduct street-based outreach in parks, transportation hubs, encampments, and other locations where unsheltered individuals may be residing.
- Build trusting relationships through consistent, compassionate, trauma-informed engagement.
- Respond to PATH HOME alerts and community service requests, prioritizing outreach based on participant needs.
- Utilize Motivational Interviewing and evidence-based engagement strategies to encourage voluntary participation in services.
- Distribute food, water, clothing, hygiene supplies, blankets, and other essential resources.
- Assess participant needs related to housing, behavioral health, substance use treatment, medical care, and other support services.
- Recognize and respond appropriately to medical emergencies, psychiatric crises, overdoses, and other urgent situations, including administering naloxone (Narcan) when appropriate.
- Coordinate referrals and warm handoffs to healthcare providers, behavioral health treatment, shelters, housing agencies, and community partners.
- Provide transportation to appointments and community resources when appropriate.
- Maintain timely, accurate documentation in HMIS and other required systems while ensuring compliance with HIPAA and agency policies.
- Participate in supervision, staff meetings, training, and quality improvement activities.
- Perform other duties as assigned by manager.
Required Education and Experience:
- High school diploma or equivalent required.
- Minimum one (1) year of experience working with individuals experiencing homelessness, substance use disorders, mental health conditions, co-occurring disorders, or related social service needs.
- Individuals with relevant lived experience are encouraged to apply.
- Community Health Worker (CHW) or Certified Peer Recovery Specialist (CPRS) credential preferred.
Required Licensing or Certifications:
- Narcan certification within 30 days of hire.
- CPR certification within 30 days of hire.
- Valid New Jersey driver’s license with an acceptable driving record.
Specialized Skills, Abilities and Knowledge:
- Knowledge of Motivational Interviewing, trauma-informed care, and low-barrier engagement strategies.
- Ability to recognize and respond effectively to crisis situations.
- Experience using HMIS or other case management systems.
- Knowledge of community resources and homeless service systems preferred.
- Training in Opioid Overdose Prevention and Mental Health First Aid is a plus.
- Strong communication, organizational, and problem-solving skills.
- Bilingual English/Spanish preferred.
Work Schedule: This position is available on a full-time or part-time basis. The Street Outreach and Engagement Program operates seven (7) days per week from 4:00 PM to 12:00 AM. Schedules will be determined based on organizational needs.
