Program Manager – Mobile Community Care

We are excited to announce our Program Manager! The Program Manager is responsible for the management of a mobile health clinic designed to reduce barriers to medications for substance use disorder, particularly opioid use disorder, and other essential health services. Mobile Community Care travels to areas in Newark, NJ that have experienced high rates of overdoses and/or have high numbers of individuals experiencing homelessness to provide low-threshold medication services, harm reduction services, and care coordination services that promote improved health and well-being.

Primary Duties and Responsibilities of a Program Manager:

Program Administration:

  • Monitor service delivery and ensure compliance with New Jersey Division of Mental Health and Addiction Services (DMHAS) Annex A requirements.
  • Identify service areas and create/maintain schedules to ensure the provision of services to at least 100 individuals per year.
  • Collaborate with a range of system partners to ensure coordination of services and continuity of care including pharmacies, substance use treatment providers, opioid treatment programs (OTPs), office-based addiction treatment (OBAT) programs, mental health service organizations, federally qualified health centers (FQHCs), primary care providers, harm reduction service providers, and other entities.
  • Ensure the timely and accurate submission of all funder reports and requests.
  • Ensure required data and documentation are entered into electronic health record through direct entry and collaboration with team members.
  • Ensure program adheres to federal and state licensing and accreditation standards and requirements.
  • Provide administrative supervision/support for direct reports, including training and evaluation.
  • Coordinate marketing activities in collaboration with the Marketing Manager.

Direct Services

  • Conduct screenings for substance use disorder and social determinants of health.
  • Screen community members for insurance membership and assist them in enrolling in Medicaid using presumptive eligibility whenever possible.
  • Provide care coordination services, including referrals to primary care, mental health services, specialty services, gynecological services, etc.
  • Coordinate with affiliated pharmacies to ensure prescriptions are filled in a timely manner.
  • Collaborate with team members to provide education on harm reduction strategies and practices.
  • Offer supportive services/case management to individuals experiencing challenges in participating in the program or transferring to other medication prescribers.
  • Follow up with staff at affiliated agencies for at least three (3) months to ensure continuity of medication services.
  • Deliver services in a manner that aligns with National Cultural and Linguistically Appropriate Services (CLAS) standards.
  • Collect required data sets.
  • Ensure client satisfaction by soliciting feedback and satisfaction surveys.

Required Education and Experience:

  • Bachelor’s degree required.
  • 2-4 years of experience in behavioral health, human services, or public health.
  • Management/supervisory experience preferred.

Required Licensing or Certifications:

  • Relevant licensure (e.g., LSW, LCSW, LCADC, LPC), preferred but not required.

Specialized Skills, Abilities and Knowledge:

  • Understanding of substance use disorder and harm reduction philosophy.
  • Ability to handle crises and quickly adapt/pivot when necessary.
  • Ability to learn and efficiently use the electronic health record.
  • Ability to demonstrate cultural awareness, genuine positive regard and empathy regarding client concerns.
  • Demonstrated ability to maintain strict confidentiality in performing duties.
  • Project management experience a plus.

Schedule: Monday through Friday, 9:00am-5:00pm
