We are excited to announce our Case/Care Manager opening in Paramus, NJ!

Primary Duties and Responsibilities of a Case/Care Manager:

Key Responsibilities:

  • Plans effective referrals and aftercare follow-up processes to ensure the continuum of care.
  • Gather and collect information to assist and support the clinical team members in completing aftercare referral forms that addresses risk factors, substance abuse history, mental health history, psychosocial history, community functioning, performance outcomes, strengths, challenges, goals, interventions, and discharge planning.
  • Implements and manages Integrated Care Plan by linking and engaging clients with services identified as necessary to address the individual client needs, including medical, nutrition, substance abuse, STD/HIV, education, and vocational training.
  • Actively sets priorities, implementing strategies, evaluating outcomes, and ensuring coordination of continuing care activities.
  • Attend Treatment Planning Meetings on the Addiction units to collaborate with the hospital’s medical team and clinicians. Assist and support the clinical staff when developing clinical approaches to address patient needs.
  • Serve as a liaison to the Integrity House Centralized Admissions Department when clients are being referred to and from Integrity.
  • Enter all clients transferring to Integrity House into the CORE (EHR system) and complete all merge mail letters/documentation in addition to all NJSAMS tasks; Registration, Immediate Need Profile, UNCOPE, Income/Program Eligibility, Assessment Authorization (when applicable), Admission module, Submission for IME clinical review and Treatment Authorization module.

Education/Experience:

  • Bachelor’s Degree in Counseling or Social Work (Master’s Preferred)
  • At least 2 years of experience in the Addictions and/or Mental Health field, preferred
  • Knowledge of ASAM criteria
  • Proficient with Technology
  • Must possess good written and verbal skills

Required Licensing or Certifications:

  • Clinical Licensure preferred but not required (e.g., CADC/LCADC, LSW/LAC)
  • Valid Driver’s License (Travel between Newark and Secaucus Facilities, when needed)

Work Schedule: Sunday through Thursday – 10:00am to 6:00pm

Physical/Medical Requirements: Before hire, candidates shall be required to show documentation of COVID-19 vaccination and booster and have a negative TB skin test and positive titers for the MMR.
